Is Sumter Family Health Center part of Palmetto Health Tuomey?
NO! SFHC is a Community Health Center (CHC). A CHC is a community-based, non-profit organization that provides comprehensive, high-quality, patient-focused health care services. With a focus on primary care, prevention, education, and case management, CHCs serve the primary health care needs of more than 22 million patients in over 9,000 locations across the United States.
What makes SFHC different from other medical practices?
OUR MISSION. As a federally-funded Community Health Center, SFHC’s mission is to be a comprehensive primary health care practice that is dedicated to building a healthy community and improving the individual health, well-being, and quality of life of each person we serve. We provide care for the entire family including: family practice, pediatrics, behavioral health & counseling, OB/Gyn & Midwifery and infectious disease services. We also offer additional services such as: on-site pharmacy (Drive Thru), translation & transportation services, financial discount programs to those who qualify, case management and assistance with free prescriptions. Finally, we have affordable payment options including discounts for the uninsured. We accept most insurances including Medicare and Medicaid.
What type of doctors do you have on staff?
STAFF OF INTERDISCIPLANARY PROVIDERS. The SFHC providers offer decades of experience providing the highest quality healthcare for the entire family. Our providers are a unique mix of highly qualified board certified physicians, nurse practitioners, physician assistants, Certified Nurse Midwives, a physiatrist and licensed social workers and counselors. We also have Licensed Social and Case Workers, Infectious Disease Specialists, an On-site Medicaid Eligibility Assistant, Interpreters, Certified Billers, Pharmacists and Certified Public Health Assistants on staff. Our dedicated staff works together to provide the highest quality care in a supportive and nurturing environment for your entire family.
Is SFHC a Free Clinic?
NO. We do not offer free services. A sliding fee discount is available to patients who qualify, based on income and family size. You may apply for the sliding-fee discount during registration or you can print out the form and bring the completed application to your first visit. Please see “How to Become a New Patient”. This will also provide the list of documentation you will need to bring to prove income and household information for the application.
What are my Payment Options and Methods?
SFHC accepts most private insurances (See List), Medicaid, Medicare, Tricare and sliding fee discounts for those without insurance and who qualify. SFHC accepts Cash, Check, Visa, MasterCard and money order as modes of payment.
Can I see the same doctor every time?
YES. SFHC wants to be the medical home for you and your family. Our goal is to provide each patient high-quality care in a patient focused manner. Therefore, we want you to feel comfortable with your doctor and be active in your own care. We want to equip our patients with the knowledge, ability, and motivation to make healthy choices and live healthy lives and part of that is trusting in the care you receive and who provides it at each visit.
Can I change doctors if I want to?
Yes. Your appointments are scheduled with the same provider. However, you may change providers at any time. Just let the receptionist know at the time you are making your appointment.
What if I get sick after hours or on the weekend?
For unexpected problems or illnesses, we offer round-the-clock coverage for your health care problems. If you need to reach a health care provider after hours, patients should call the Health Center’s main telephone number (803.774.4500) and the appropriate number will be provided.
What if I need to go to the emergency room?
If you are experiencing a serious or life-threatening emergency, call 911 or go the nearest Emergency Room.
What if I get sick, but do not have an appointment?
Sumter Family Health Center offers a"Work-In" Family Care program without an appointment. We offer services for minor injuries, sudden aches or pains or minor illnesses. You may be worked in with another provider depending on their schedules. However, our commitment to you remains the same and all providers give our patients and their families the same quality health care you expect when you come in for an appointment. Due to the number of work-in appointments each day these services are only available for our SFHC patients.
What if I need to be hospitalized?
If you or your family member must be admitted to the hospital, SFHC can provide your medical records to the hospitalist on staff.
Why should I go to SFHC instead of any other doctor's office if I have insurance?
The SFHC providers and staff will provide you with the excellent healthcare to all of our patients whether they have insurance or not. Our providers have decades of experience and provide care to patients of all ages. We offer the convenience of an in-house laboratory and pharmacy to save you time and money. If you were to lose your health insurance, you will never be turned away. You can expect the same compassionate and consistent care that you have always received and our staff would help you apply for our sliding fee discount program. You can also make a difference in the healthcare of an uninsured patient as well. Uninsured patient in our sliding fee program pay what they can, which is most often less than the cost of care. The income received from serving insured patients offsets the cost of caring for those less fortunate patients.
