How to Become a Patient of SFHC

Make An Appointment

To make an appointment, please call the health center location where you would like to get your care (click here for locations and phone numbers). Our operators are happy to assist you!

For emergencies and unexpected problems, we offer round-the-clock coverage for your health care problems. If you need to reach a health care provider after hours, patients should call the Health Center’s main telephone number (803.774.4500) and the appropriate number will be provided.

If you are experiencing a serious or life-threatening emergency, call 911 or go the nearest Emergency Room.

First Visit

Please plan to arrive 15 – 30 minutes prior to your appointment. This will allow our registration staff time to insure they have all the information needed for your medical records to provide you with quality care and financial assistance as needed.

No Insurance... No Problem
Bring the following items to your first appointment:

Once we have reviewed this information, we will assist you in filling out an application for financial assistance. There is a $25.00 minimal fee for each visit, regardless of the discount given. If you do not have insurance, you may also qualify for medication assistance. (Spanish Application)

Payment Arrangements

SFHC accepts most private insurances including Medicare and Medicaid (also Medicaid HMO's). If there is a co-pay or deductible for your plan, payment is expected at the time of service.

If you're uninsured or underinsured (annual deductibles), a sliding scale is provided based upon family size and income which determines the rate of charge. We will work with you to set up a payment plan; however, we do request that you pay the $50.00 minimum at your initial visit. If you qualify for the sliding fee we ask that you bring a minimum of $25.00 to each visit thereafter.

Follow-Up Visits
“A government issued photo ID and insurance card will be required at every visit.”

It is important that you schedule necessary follow-up appointments, and that you show up for your appointments on time. As an established patient, work-in appointments are accepted as the schedule allows.
