Infectious Disease Care
As part of the Midlands Care Consortium (MCC), based in Columbia, SFHC offers HIV and infectious disease services to many patients in the Sumter County, Lee and Clarendon area. Up until 2002, patients had to travel to Columbia for these services. Due to the large number of patients needing services, as well as severe transportation issues, the partnership between Sumter Family Health Center and MCC developed.
Our providers specialize in the treatment of HIV. Along with an experienced SFHC team, they not only provide medical follow-up for each of the patients, but they also provide case management for every patient. They also help the patients with medication adherence. The Infectious Disease Team has a vast knowledge of HIV/AIDS and they genuinely care about the patient’s needs. Our patient’s needs are met from the time they walk in the door to the time they leave and beyond. The patients become more than patients and are treated like family.
In 2012, SFHC partnered with DHEC for a FREE HIV Testing grant. Testing is performed daily and at events in the community throughout the year, as well. The team also provides Linkage to Care including tracking and reporting of the results of this testing.
The SFHC Infectious Disease team includes infectious disease physicians, a licensed independent social worker (LISW), case managers, social workers, RN and public health assistants, as well as clinical and support staff.
